Lyons Primary Care Group in Greystone, AL is seeking a dedicated and compassionate LPN to join our team. The ideal candidate will play a vital role in delivering high-quality, patient-centered care in a primary care setting. This position involves working collaboratively with other healthcare professionals to ensure the optimal health and well-being of our patients.

Qualifications

• Active LPN license in Alabama.
• Associate Degree in Nursing (ADN)
• Minimum of 1-2 years of nursing experience, preferably in a primary care, outpatient, hospital, family medicine, or similar setting.
• Strong communication and organizational skills.
• Ability to multitask and maintain attention to detail in a fast-paced environment.
• Proficient in electronic medical record (EMR) systems.
• Strong phlebotomy and injection skills.
• Successful completion of nursing training and certification as a nurse from an approved program.

Job Summary & Responsibilities

• Provide direct care to patients, including assessments, treatments, and follow-up care, based on individualized plans.
• Drawing blood/venipunture, obtaining specimen for lab work
• Medication injections
• Document patient history, symptoms, and health insights accurately in medical records.
• Providing scribing and data entry for providers as needed.
• Administer medications and vaccinations safely, in adherence to physician orders and best practices.
• Assist with minor procedures, wound care, and routine medical interventions as required.
• Educate and guide patients on managing chronic illnesses, preventive care, and healthy lifestyle practices.
• Coordinate with multidisciplinary teams to ensure continuity and quality of care.
• Monitor patient progress and proactively address any health concerns or complications.
• Maintain compliance with all regulatory health and safety guidelines.
