Role specifics and benefits: As a Home Health RN, you will assess patients in their homes, develop and adjust individualized care plans, perform wound care, administer medications, monitor vital signs, manage IV therapies when appropriate, and coordinate with physicians, therapists, and social work to align goals with patient priorities. You will educate patients and families on disease management, safety at home, nutrition, and medication adherence; document care accurately in electronic medical records; and contribute to quality outcomes through careful data collection and timely communication. The position includes competitive compensation with a weekly pay range of 1,992 to 2,032, and guaranteed 40 hours per week for the contract length.
