Denver, Aurora, Littleton, Englewood, Lakewood, Highland Ranch, Commerce City, Arvada, Westminster, Qualifications: Must have good standing RN licenseCurrent CPR certificationKnowledge/skills with short skilled nursing visitsMust be willing to provide in home nursing care Education and Experience Requirements for Registered Nurse (RN): One to one patient care in a home environmentExecution of physicians prescribed plan of care and compliant documentation of careAdministration of prescribed medication, treatments and therapiesPatient assessments, SOC and follow up visits, coordination of care with physician and other disciplinesAbility to work independentlyExperience with electronic documentation systemsProvide effective communication to patient/family, team Responsibilities: One to one patient care in a home environmentExecution of physicians prescribed plan of care and compliant documentation of careAdministration of prescribed medication, treatments and therapiesPatient assessments, works closely with RN and carries the POC, coordination of care with physician and other disciplinesProvide effective communication to patient/family, teamAbility to work independentlyExperience with electronic documentation systems Qualifications: Required RN License Home & community care experience Preferred CPR Certification Document management systems RN experience RN experience (2-5 years) Additional notes: This job requires a background check.
