The Haven Texas is a provider of detox and stabilization services for individuals experiencing substance abuse withdrawals. Our 5-7 day programs are located within acute care hospitals and offer comprehensive treatment options. We accept Medicare, Medicaid, and are in-network with all major insurance providers. Additionally, we provide 24-hour admission and transportation services for our clients.
Role Description
Do you have a penchant for organizing, collaborating, smiling, and having fun? We are a medical stabilization unit located in a prominent Dallas hospital. Our team provides support and medical care to those patients undergoing alcohol and drug withdrawals, among other medical complications.
We are currently seeking a Discharge Coordinator / Discharge Planner. The role involves continuous assessment of the patient's status and thorough documentation of pertinent information. It also requires accurate documentation of communication and coordination for the patient's discharge and transition to the next level of care. The incumbent will serve as an information resource to patients and maintain consistent and effective communication with all departments within the unit. Additionally, they will be responsible for matching resources and referrals to meet the needs of the patients.
*One year or more of experience in discharge planning or supporting patients transitional care is preferred (but not required)
* Individuals in recovery with at least one year of sobriety are strongly encouraged to apply
Licenses, Registrations, Certifications, or Special Requirements:
Licensed Chemical Dependency Counselor (LCDC); or
Licensed Professional Counselor Associate (LPC-Associate); or
Licensed Master Social Worker (LMSW)
Valid Texas Driver's License.
Preferred:
Bilingual in English and Spanish.
