Job Description

The Family Wellness Liaison (FWL), Bilingual will provide support maternal child health resources and initiatives. The FWL will support patient’s and families receiving health within programming through improving health care utilization, advocates on their behalf and works to reduce barriers that interfere with the patient's ability to successfully interface with their medical home provider. The FWL will develop and facilitate group-based health education programming in clinic and community settings as it relates to prenatal, birthing, postpartum, interconception, positive parenting, breastfeed care and more topic areas.

Typical Duties

• Completes health risk screenings in accordance with grant initiative protocols and evaluation procedures established by the data team Program Manager.
• Thoroughly documents and reports all activities in the grant tracking software according to data and evaluation teams as well as the Program Managers established policy and procedure.
• Consults information systems such as EMR and Cerner, Referral systems, RedCap, t coordinate and manage care and to assess for eligibility for services including patient’s eligibility for service.
• Enters all assigned risk screening assessments into the designated databases.
• Performs supportive tasks for members such as scheduling appointments, referrals to community-based resources and directions on how to access services and care, follows up to close gaps in care.
• Works under the direction of the Program Manager to link participants to appropriate programs.
• Supports efforts to locate Program Participants, prenatal patients, or families lost in care to maintain updated records and documentation
• Completes all education activities/training as required by HRSA, State of Illinois.
• Provides support to strengthen the Consortium and birth equity Initiatives through active engagement and participation, assistance with Consortium content, logistics, planning and recruitment of participants
• Develops and facilitates male (partner) centered events
• Plans content and facilitates Consortium and birth equity meetings on a rotation schedule
• Coordinates home visits with Doula to patients assigned and shares care plans to alleviate the
• patients' barriers to care and their recovery.
• Maintains postpartum contact every two (2) weeks up to twelve (12) weeks post-delivery and ensures patients have longitudinal PCP care assigned.
