Dementia Care Advisor - Program Coordinator
Location: Indianapolis, Indiana – this position is a hybrid position but needs to be located in Central Indiana to be onsite with partners as needed.

Job Summary:
We are seeking a Dementia Care Program Coordinator to work alongside our partners to support PathWays members and their informal caregivers affected by Alzheimer’s and other related dementias. This tailored, person-centered service offering will provide the education, resources, and support to those affected by dementia to minimize common challenges inherent to the condition, maximize health outcomes and independence of the member with dementia, and improve caregiver proficiency while reducing caregiver stress and burden.
The Dementia Care Program Coordinator will identify, research, validate, build, and execute impactful health programs and initiatives. The role will be responsible for supporting multiple efforts, including conducting comprehensive research, building relationships with members and caregivers, designing new programs, and supporting the launch and execution of new initiatives.
