We serve adults with serious mental illness—many navigating housing instability, justice involvement, or co-occurring conditions—by delivering wraparound services that are mobile, person-centered, and always available.

About the Role

As an ACT Case Manager, you’ll provide direct support and case management to clients in their homes, shelters, or wherever they are. You’ll collaborate closely with therapists, psychiatric providers, and peer specialists to coordinate care, reduce hospitalizations, and promote long-term recovery.

What You’ll Do

  • Deliver intensive case management services to adults with SPMI in home and community settings.
  • Conduct routine field visits across the Chicagoland area to assess, engage, and support clients.
  • Help clients access mental health care, housing, benefits, medical providers, and recovery services.
  • Collaborate remotely with ACT team members: psychiatrists, nurses, therapists, and peer specialists.
  • Maintain up-to-date documentation on client progress, interventions, and treatment goals.
  • Assist with medication support, crisis prevention planning, and skill-building activities.
  • Complete IM-CANS assessments and help develop person-centered service plans
  • Promote autonomy and recovery by meeting clients where they are—physically and emotionally.
  • Build trust through frequent, in-person contact with a small caseload.

What You Bring

✔ Bachelor’s degree in Social Work, Psychology, Human Services, or related field

✔ 2+ years of experience in case management, mental health, or community outreach

✔ Strong communication skills and a commitment to client dignity and autonomy

✔ Comfort navigating Chicago neighborhoods and working in nontraditional settings

✔ Ability to remain calm and grounded during psychiatric or behavioral crises

Bonus if you’re:

➕ Familiar with the ACT model or community-based mental health

➕ Bilingual (Spanish or Arabic preferred)

➕ Experienced with Medicaid/SSDI systems or the IM-CANS tool

Location: Field-based across Chicago, with a home base at our Gage Park or Bucktown clinic

Schedule: Monday–Friday, daytime hours, some flexibility required

Compensation: $22-$26/hour + mileage reimbursement + benefits
